Patient Intake Infrastructure
The appointment scheduler below is an example of the patient experience we deploy directly on a physician or practice website. Patients can access the intake through a simple URL such as YourPractice.com/appointment. We can customize the form around your practice’s physicians and their schedules, appointment types, workflows, and scheduling process. Try the demonstration below. Once a patient schedules the appointment, they would be directed into the next stage of the telehealth workflow which is reminders for their visit. Calendar availability can be synchronized with the practice’s approved calendar environment during onboarding.

Returning Patients: If your provider or practice instructed you to schedule a follow-up visit, you may continue directly to scheduling.
Appointment availability can be synchronized with the practice’s approved calendar environment during CHA onboarding. Existing calendar commitments can automatically be excluded from available appointment times.
